Why Does Eyelid Skin Sag?

Collagen and elastin deplete with age, the orbicularis oculi muscle thins, and orbital fat shifts. Because eyelid skin is the thinnest on the body, it sags early and noticeably. Sun exposure, smoking, eye rubbing, and genetics accelerate this process. As a result, the eyelid crease becomes hooded, the lash line is weighed down, the eyes look tired and smaller, and some people experience eyelid heaviness toward the late afternoon.

Three Easily Confused Conditions

Dermatochalasis (excess skin): surplus skin hoods the crease, but the eyelid margin remains in its correct position; lifting the skin allows the eye to open normally. Ptosis (drooping eyelid): the eyelid margin droops over the pupil due to levator muscle dysfunction. Brow ptosis (sagging brow): the eyebrow descends below the orbital rim, pushing excess skin onto the eyelid; lifting the brow resolves the hooded eyelid skin. Performing only an upper blepharoplasty on someone with brow ptosis will cause the eyebrow to drop even further; performing only skin excision on someone with true ptosis will leave the eyes looking perpetually tired.

How Blepharoplasty for Middle-Aged Patients Differs from Younger Patients

The goal is not to create a high, prominent crease, but to excise the precise amount of excess skin, preserve a natural crease, conservatively address herniated fat to prevent a sunken look, and evaluate the levator muscle. Removing too much skin or fat leads to hollow, sunken eyes that appear prematurely aged—a common pitfall. The surgeon measures the excess skin on each side individually, as asymmetry is very common.
