Why Do Sunken Eyes Make You Look Tired?
Around the eyes, the play of light and shadow defines whether you look refreshed or fatigued. When periorbital fat atrophies or shifts, the upper eyelids become hollow, creating a deep groove beneath the brow bone. In the lower eyelids, the junction between the lid and cheek becomes pronounced as a tear trough, casting a shadow where light cannot reach. This shadow is often mistaken for pigmentary dark circles; however, concealer cannot mask it because the issue is anatomical, not discoloration.
Three Common Patterns of Volume Loss
Congenitally deep-set eyes: A deep orbital bone structure causes upper eyelid hollowing from a young age, often accompanied by prominent double eyelids. Age-related fat atrophy: Upper eyelids gradually become hollow after age 35–40, while brow ptosis further deepens the crease. Under-eye bags with tear troughs: Lower eyelid fat herniates forward to form bags, directly above a sunken tear trough—creating both an excess and a deficit in the exact same area. This third pattern is why simply "removing bags" or "filling the trough" alone yields incomplete results.
Fat Repositioning vs. Dermal Fillers: What’s the Difference?
Dermal filler injections temporarily restore volume and are best suited for mild hollowing without eye bags; however, because lower eyelid skin is delicate, superficial placement can cause visible lumps or a bluish tint (the Tyndall effect). Fat repositioning is a surgical procedure that shifts your own herniated lower eyelid fat into the tear trough, resolving both the bags and the hollow in a single step with long-lasting results since it uses autologous tissue. Autologous fat grafting, harvested from another donor site on your body, is used for deep hollowing without bags; because a portion of grafted fat naturally absorbs over time, the surgeon may slightly overcorrect.
What Determines the Degree of Improvement?
The degree of volume loss, the presence of eye bags, lower eyelid skin thickness, eyelid laxity, and age. Thin skin shows significant improvement in shadowing once a supportive fat layer is placed beneath it, while significant skin laxity may require concurrent skin excision.
